چکیده

Purpose: Phase I clinical trials are generally conducted to identify themaximum tolerated dose (MTD) or the biologically active dose (BAD) using a traditional dose-escalation design. This designmaynot be applied to cancer vaccines, given their unique mechanism of action. The FDA recently published "Guidance for Industry: Clinical Considerations for Therapeutic Cancer Vaccines." However, many questions about the design of cancer vaccine studies remain unanswered. Experimental Design: We analyzed the toxicity profile in 239 phase I therapeutic cancer vaccine trials. We addressed the ability of dose escalation to determine the MTD or the BAD in trials that used a dose-

Cancer Therapy: Clinical The c-Met Tyrosine Kinase Inhibitor JNJ- 38877605CausesRenal Toxicity through Species- Specific Insoluble Metabolite Formation

Purpose: The receptor tyrosine kinase c-Met plays an important role in tumorigenesis and is a novel target for anticancer treatment. This phase I, first-in-human trial, explored safety, pharmacokinetics, pharmacodynamics, and initial antitumor activity of JNJ-38877605, a potent and selective c-Met inhibitor. Experimental Design:We performed a phase I dose-escalation study according to the stand...

Dose escalation with overdose control using a quasi-continuous toxicity score in cancer Phase I clinical trials.

Escalation with overdose control (EWOC) is a Bayesian adaptive design for selecting dose levels in cancer Phase I clinical trials while controlling the posterior probability of exceeding the maximum tolerated dose (MTD). EWOC has been used by clinicians to design many cancer Phase I clinical trials, see e.g. [1-4]. However, this design treats the toxicity response as a binary indicator of dose ...

Cancer Therapy: Clinical Phase I Study to Assess the Combination of Afatinib with Trastuzumab in Patients with Advanced or Metastatic HER2-Positive Breast Cancer

Purpose: The HER2 mAb, trastuzumab, is a standard therapy for patients with HER2-positive breast cancer before acquired resistance. Afatinib, an irreversible, oral, small-molecule ErbB family blocker, shows clinical activity in trastuzumab-refractory HER2-positive breast cancer. Experimental Design: This phase I study used a 3þ3 dose escalation to determine the MTD of oral once-daily afatinib i...

Cancer Therapy: Clinical Phase I Study of Vorinostat as a Radiation Sensitizer with I-Metaiodobenzylguanidine (I-MIBG) for Patients with Relapsed or Refractory Neuroblastoma

Purpose: I-metaiodobenzylguanidine (MIBG) is a radiopharmaceutical with activity in neuroblastoma. Vorinostat is a histone deacetylase inhibitor that has radiosensitizing properties. The goal of this phase I study was to determine the MTDs of vorinostat and MIBG in combination. Experimental Design: Patients 30 years with relapsed/refractory MIBG-avid neuroblastoma were eligible. Patients receiv...
